which should accompany each Collective Bargaining Agreement as well as the
Employer Form for pre-tax withholdings.
Pre-tax contributions allow participants to supplement the contribution made by
the employer towards the cost of your employee's insurance cost. Pre-tax
election forms may only be accepted for new hires and during the Open
Enrollment Period. These forms may be obtained from the plan and may also be
found in the "Member On-Line" section during Open Enrollment. In order for a
pre-tax form to be valid for a calendar year, it must be RECEIVED by 12/31 for
the upcoming year.
